Thank you for providing your information to request a quote for property and casualty insurance for your church with UMI.  A friendly licensed agent from AmVenture Insurance Agency will contact you soon to gather information about your church and provide you with quotes from our carrier network. 

All of our carriers specialize in insurance for churches and faith-based organizations.  In order to provide you with an accurate quote for your church, we will need information regarding your church’s characteristics, operations and property.  Please be prepared to provide the following information when you speak with one of our licensed agents:




  • Legal name of the church

  • Primary contact name, phone number and email address

  • Mailing address, phone number and email address of the church

  • Number of full-time and part-time employees

  • Annual payroll for each employee

  • Benefits offered to church employees

  • Number of members in the congregation

  • Average weekly attendance

  • Church tax id or FEIN

  • Number of insurance claims or losses in the past 3 years – if any, be able to provide details and a ‘loss run’ from the current insurance carrier or agent

  • Have any of your organization's past or present employees, volunteers or representatives ever received a report, a complaint, an allegation, ever been charged, convicted, had a claim for damages submitted against, or sued in civil court for any type of sexual misconduct?

  • Does your church have a Safe Sanctuary program in place?




  • Is there a school, preschool and/or daycare?  If so, number of students, teachers, etc.

  • Is there a commercial kitchen?  If so, is there a ‘soup kitchen’ or other regular offering?

  • Is there a thrift store or other similar operation? If so, details will be needed.

  • Does the church own or operate a cemetery?  If so, is it at a separate location from the main church?  Details will be needed.

  • Does the church operate a camp?  If so, is it day or overnight?  Details will be needed.

  • Does the church own any vehicles?  If so, details will be required.

  • Are any buildings designated as historical landmarks?



  • Number of locations

  • Number of buildings per location

  • For every building at every location:

    • Physical address

    • Description (Sanctuary, Parsonage, Fellowship Hall, Garage, Shed, Playground, Education, etc.)

    • Year built

    • Number of stories

    • Foundation type (basement, crawl space, slab, etc.)

    • Square footage

    • Property value (replacement cost)

    • Contents value (include high value items like stained glass, organs, etc.)

    • Type of heat (forced hot air, boiler, solid fuel, etc.)

    • Construction type (frame, metal, masonry/brick, etc.)

    • Roof material (asphalt shingles, metal, wood, clay tile, etc.)

    • Age of roof

    • Percentage with sprinklers

    • Fire alarm

    • Security/burglar alarm

    • What year was the electrical and wiring last updated?

    • What year was the HVACV last updated?

    • What year was the plumbing last updated?
